Job Description:

dedicated Per Diem Registered Nurse (RN) Case Manager to join our
team on an as-needed basis across our extensive network of
healthcare facilities. This role involves managing patient care to
ensure optimal outcomes, effective resource utilization, and
quality service delivery. This position is ideal for individuals
looking to work on a flexible schedule while contributing to our
mission of care and service improvement. Assess new patient cases
and develop effective care plans in accordance with their health
needs. Coordinate with healthcare professionals to ensure that care
plans are implemented effectively. Monitor patient progress and
adjust care plans as necessary. Educate patients and their families
on disease processes, treatment, and home care strategies. Ensure
compliance with all healthcare regulations and standards. Strong
clinical skills in nursing Excellent communication and
interpersonal skills Proficient in healthcare management systems
Ability to work independently and in team settings Strong
organizational and problem-solving skills Medical Insurance Dental
Insurance Vision Insurance Life Insurance 401k Current Registered
Nurse license Certification in Case Management (CCM) preferred but
not required.
