Operation PAR, Inc. is a non-profit organization offering the full
continuum of services for individuals with substance use and/or
mental health disorders. PAR has been providing comprehensive
behavioral health treatment in west Florida since 1970 through
advocacy, education, clinical services, and research. BENEFITS: 9

DESCRIPTION: The Program Coordinator of the Endocarditis Program is
responsible for overseeing the day-to-day clinical and operational
functions of a specialized inpatient residential program serving
individuals recovering from endocarditis. This role ensures quality
patient care, regulatory compliance, and effective coordination
among nursing staff, support teams, and external providers. The
ideal candidate will have a background in home healthcare services,
clinical experience, and strong organizational and leadership
abilities. ESSENTIAL JOB FUNCTIONS: Program Operations Coordinate
daily operations of the Endocarditis Program, ensuring smooth
patient admissions, care delivery, and discharges. Monitor census,
documentation, and staff scheduling to maintain effective workflows
and coverage. Serve as a liaison between patients, staff, and
leadership to support program goals and resolve issues promptly.
Clinical Oversight Oversee clinical staff, ensuring adherence to
clinical protocols and quality standards. Monitor patient care
plans and collaborate with the interdisciplinary team to support
holistic recovery and treatment goals. Provide hands-on clinical
care as needed within the scope of practice. Conduct regular
supervisory meetings, case reviews, and performance evaluations.
Ensure staff adherence to ethical standards and best practices in
behavioral health. Assist case manager to identify and coordinate
appropriate community resources for client care. Compliance and
Quality Assurance Ensure program adherence to state and federal
regulations, licensing requirements, and organizational policies.
Assist with audits, incident reporting, and continuous quality
improvement initiatives. Monitor documentation practices and
support staff in maintaining accurate records within the electronic
health record (EHR) system. Staff Support and Supervision Provide
guidance, support, and supervision to nursing and direct care staff
assigned to the program. Participate in onboarding and training of
new staff, with a focus on endocarditis care protocols and
infection control. Offer ongoing coaching and feedback to promote
professional growth and performance Patient and Family Engagement
Foster a supportive, trauma-informed care environment for patients
throughout their recovery journey. Communicate with family members
or caregivers as appropriate, coordinating home healthcare services
and discharge planning Community Coordination Collaborate with home
health agencies, hospitals, and referral partners to ensure
continuity of care and appropriate follow-up services Support
transitions to outpatient care or home healthcare as part of the
discharge process Core Competencies Clinical Judgment: Strong

fast-paced, patient-centered environment EDUCATION AND EXPERIENCE:
Master's degree in Social Work, Counseling, Psychology, or a
related field. Must hold either current licensure as a Licensed
Clinical Social Worker (LCSW), Licensed Professional Counselor
(LPC/LMHC), or MCAP/CAP. Minimum of three (3) years of experience
in behavioral health, including supervisory experience. Experience
Preferred: Minimum of 1 year of clinical medical experience.
Experience in home healthcare services is strongly preferred.
Experience in a residential or inpatient setting is preferred.
SPECIAL REQUIREMENTS: Valid driver's license Must pass level II

maintain current training as required by Operation PAR, Inc.
policies Must maintain active/clear RN license in the state of
Florida ESSENTIAL PHYSICAL/TECHNICAL SKILLS: Ability to write
accurately and legibly Moderate proficiency in the use of Microsoft
Windows-based computers Ability to accurately type at least 40
words per minute Ability to sit at a desk and view a display screen
